Private Care Management

Our care management services are designed to help individuals and family members find the services they need to remain living independently in their own homes. Our experienced social workers respond quickly to inquiries and provide the initial consultation at no fee. Services thereafter are billed monthly at an hourly rate. While this service is not paid for by Medicare or insurance, our staff provides consultation on maximum use of benefits that are available.
