Contract Case Management

There are times when the mandated provider of case management services is unable to do so because of staffing or geographical issues. Adult Care Consultants has provided case management to a variety of counties and non-profit agencies since 2000. Past and present contracts include Waupaca County Department of Human Services, Winnebago County Human Services, Marathon County Human Services, Ashland County Human Services, Shawano County Department of Community Programs and Northern Bridges.
